About the program

The Healthful Transitions Program helps support URO's goals of eliminating systemic barriers to reentry for people coming home from jail or prison by cultivating a compassionate and committed group of community volunteers, the Interfaith Based Reentry Support Team, to do the following:

  • Execute motivational interviewing, goal setting, and success planning exercises in local prisons and jails

  • Assist with transitioning participants to various medical appointments, job readiness workshops, apprenticeship programs and educational opportunities to enhance the health and future prospects of our participants

  • Participate in bi-weekly meals to check-in with new community members and fellow volunteers while learning from guest speakers and sharing healthful resources
